Community Wellbeing Officer

Salary:  £18,500 – Full time, fixed term contract (12 months)

Hours: 37.5 hours per week

Purpose of post:

We’re looking for an experienced community worker to join our dedicated team and play a crucial role in engaging Cheshire and Merseyside residents in health and wellbeing activities through GP Practices and hospitals.

The post holder will be working closely with Warrington and Halton Hospital to recruit, induct, and support volunteers through various volunteering opportunities.

The successful candidate will be working collaboratively and creatively with practice patients, staff, Clinicians and wider stakeholders to generate better health outcomes for patients.

Key responsibilities:

  • Coordinate and deliver a varied range of educational and social inclusion courses to members of the public.
  • Undertake holistic one-to-one consultations with patients that address the social determinants of health
  • Lead a volunteer programme based at Warrington and Halton Hospital, volunteers will be actively recruited, and inducted to support staff and patients.
  • To monitor and review volunteer placements to ensure volunteers receive sufficient support and achieve their goals.
  • To develop an in-depth knowledge of the assets and resources available in the area that can help local people to improve their health and wellbeing.
  • Actively participate in Multi Disciplinary Meetings, Patient Participation Group meetings and attend wider networking opportunities; developing opportunities to work with others and supporting integrated working
  • Play a crucial role in the monitoring and attainment of key targets for Wellbeing Enterprises’ contracts, using a range of validated qualitative and quantitative methods
  • Attend local network meetings to promote an awareness of the project and to identify opportunities for partnership working
  • To collate evaluation data to measure a range of health and social outcomes of people who make use of the service.
  • Undertake administrative duties associated with the post, for example data-inputting, telephone evaluation and producing promotional materials.
