Wellbeing Enterprises (WE) is expanding its team and recruiting a Children & Young People (CYP) Link Worker to work with health and care teams and the voluntary sector to provide 1:1 personalised support to CYP and their families in East Cheshire who self-refer or are referred to them.
This post aims to help CYP and their families to take control of their health and wellbeing by giving time to focus on ‘what matters to them’. The CYP Link Worker will build trusting relationships with CYP, create a shared personalised care and support plan and connect them to local, diverse and culturally appropriate community groups, voluntary, community, faith and social enterprise (VCFSE) organisations and services. The post holder will provide 1-1 support to CYP over the phone and online while based in our Halton office, as well as face to face support and outreach work at agreed locations in East Cheshire. This will include the provision of CYP group activities.
This role helps CYP and their families to work on their wider health and wellbeing, specifically addressing access issues and the wider determinants of health as they relate to CYP such as family debt, bullying, poor housing, peer pressure and other lifestyle factors and low-level mental health concerns by increasing active involvement in their local community. The post holder will assess what’s going on for CYP that may be bothering them and develop and implement a wellbeing plan to address any barriers to health and wellbeing while helping them reconnect with their strengths and build their capabilities. This approach particularly helps people with long term conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing.
